Next recommendation... The Five Dysfunctions of a Team. It is also a quick read and laid out in the same parable style. Here are the dysfunctions:

1) Absence of trust
2) Fear of conflict
3) Lack of commitment
4) Avoidance of accountability
5) Inattention to results

I'm leading a group of nonprofits who meet monthly to discuss topics of interest and to work on collaborating together on common goals. It's not quite the same thing as your church because our mutual connection, the nonprofit world, is a bit broader than your ministry.

However, I think you may be able to adapt something that we do. Since we represent so many different groups, we have initiated a time of collaboration where one group is selected prior to the meeting. The rep. from that group prepares a brief, 5-minute presentation on the organization and focuses in particular on its challenges. As the moderator I cut that part off and kick off the collaboration time where different nonprofit leaders from the group ask questions, share insights, and offer help. I cut it off at eight minutes and we move on to our main topic for the meeting, however I always emphasize that this time is a catalyst for further discussion. We're just pitching ideas and trying to stimulate additional discussion that can take place outside of the meeting time.

Though committees can be an equally problematic affliction in addition to meetings, we're hoping that committees will form naturally without official assignments or mandates as a result of this process. You can view our group's web site at: http://nnnvt.wordpress.com.
